Personal data and medical record

It is important to Witte Kruis that personal data must be processed and safeguarded with care. It is our priority in all circumstances to comply with privacy legislation in the Netherlands (including the General Data Protection Regulation (GDPR), the Dutch Medical Treatment Contracts Act (Wet Geneeskundige Behandelovereenkomst), and the Supplementary Provisions Act (Wet aanvullende bepalingen)).

You have the right to see your own personal and medical data. You further have the right to modify, destroy or transfer your personal data.

There are 2 possibilities for you to see your medical record, depending on the region where you received ambulance care.

1) Online via the portal:
For the Zeeland region it is possible under certain conditions* as from 5 July 2021 to see your online record via www.mijnambulancezorg.nl. Other regions where Witte Kruis provides ambulance care will follow in the coming period. To access your medical record, log in with your DigiD.

*One important condition to meet before your record can be sent to the portal is that we as ambulance service must establish your identity based on a valid proof of identity (Dutch passport, identity card, driver’s licence or a foreigner’s identity card). This is a legal requirement to avoid the unwanted exchange of medical data between patients.

It can prevent your medical record from hanging online on the portal. This can happen if we were not able to do the identity check. In such cases you must not upload your record to the online portal. You will then be referred back to us from the portal to request your medical record in the following way. To do so, you must fill in this form. The form can be sent to communicatie@wittekruis.nl

2) Using the form on this website: 
Use this form to submit a request to see your medical record. 

The medical record may only be released once the identity has been verified. If you are making a request on someone else’s behalf, you must also send the authorisation form along with it
