Personal data and medical record

There are 2 possibilities for you to see your medical record.

1) Online via the portal:
You can see your online record via www.mijnambulancezorg.nl. To access your medical record, log in with your DigiD.

*One important condition to meet before your record can be sent to the portal is that we as ambulance service must establish your identity based on a valid proof of identity (Dutch passport, identity card, driver’s licence or a foreigner’s identity card). This is a legal requirement to avoid the unwanted exchange of medical data between patients.

It can prevent your medical record from hanging online on the portal. This can happen if we were not able to do the identity check. In such cases you must not upload your record to the online portal. You will then be referred back to us from the portal to request your medical record in the following way. To do so, you must fill in this form. The form can be sent to info@wittekruis.nl

2) Using the form on this website: 
Use this form to submit a request to see your medical record. 

The medical record may only be released once the identity has been verified.

Once we have received it, we will contact you to arrange the next steps and explain how we verify the identity. Use this form to submit a request to change or destroy personal and medical data. This form can be sent to info@wittekruis.nl
